Information about the District H1N1 Vaccine Clinic has been sent home with
your child. The clinic for school-age children will be held in the
cafeteria of the Nazareth Area Middle School on Thursday, November 19 from
4:00pm to 8:00pm and on Saturday, November 21 from 9:00am until noon. If
you did not receive the information, you can download and complete the
consent form from the District website at http://www.nazarethasd.k12.pa.us/.
Please park in the back of the building and enter through the rear doors by
the swimming pool.
